In 2001, HRSA/HHS awarded a "Community Access Program (CAP) grant"
in the amount of $ 992,000 to Yale-New Haven Hospital (YNHH) and its
collaborating partners: Fair Haven Community Health Center (FHCHC), the
Hill Health Center (HHC), the Hospital of Saint Raphael (HSR) and the
New Haven Health Department (NHHD). The collaborators committed
themselves to develop a program to improve health status, promote
prevention and address disparities; this program is named Wellness Information Network (WIN).
Wellness Information Network
program improves health care access for the uninsured and underinsured
residents of the City of New Haven. The WIN program focuses on keeping
people healthy and engaged in the health care system, reduces the
Emergency Department visits and admissions, and reduces duplication of
services. The uninsured and underinsured are often not connected to a
health care system.

Approach to Addressing these Challenges:
- Develop a Community Health Information Network and case management
system that will support the establishment of medical homes for the
uninsured and underinsured
- Allow the primary care providers to share information while ensuring patients' confidentialty
- Develop case management at each site to address the psychosocial and other needs of the uninsured and underinsured
- Work with each of the primary care providers to ensure referred patients remain in care
- Establish sliding fees so each patient may better afford the services
WIN 's Primary Goals:
- Allow people who are uninsured and underinsured to receive the same quality health care as people with insurance
- Ensure patients have access to health care when they need it
- Make sure each uninsured and underinsured patient has a primary care provider at a "medical home"
- Increase positive health outcomes
- Prevent illness that can lead to hospitalizations
- Decrease unnecessary duplications of medical care, because medical histories and previous test results are available when needed
- Reduce duplicated or preventable Emergency Department visits

program that is focused on expanding health care access for people in
New Haven by adding innovation to the Health Care System.
